I've also had to be a zealous advocate to get what is needed (and yes, medically necessary) under my insurance contract. With no help from the insurance company I contracted with. Actually, "no help" would have been a blessing - more like "in spite of" my insurance company's adverse position regarding their contracted-for and paid-for obligations.
